Initial Request:

To request an individual, child, couples, or family therapy appointment or to obtain additional information about any of our counseling services, please fill out the form below or call the office at (850) 243-8086. Our staff will respond to your Appointment Inquiry within 2 business days

If you call the office and staff are not available to answer your call, please leave a voice mail at the end of the recorded message. We check voicemails at the opening of each business day and periodically during a regular work day, all efforts will be made to return your call as quickly as possible.

Medical insurance must be verified prior to being seen.

Legal Evaluations (e.g., custody evaluations, court ordered drug or alcohol evaluations or treatment) are no longer being conducted by any Turning Point Provider.



Office Hours:
The office of Turning Point is open Monday - Thursday and closed on Fridays. Our telephone is answered Monday - Thursday from 8:30 AM to 4:30 PM. Therapy appointment times vary with the schedule of your therapist and may begin as early as 7:00 AM, with the last appointment being scheduled at 5:00 or 5:15 PM.


 

If You Have an Emergency:

Life Threatening Emergency:
Remember, if you have a dire or life threatening emergency and the office is closed, please go to your nearest Emergency Room for immediate assistance!

Less Severe Emergency:
If your emergency is of a less severe nature, you may call our after hours Professional On-Call by first calling the business phone number, (850) 243-8086. Following the office voice message, you will hear the number for the Professional On-Call. Notify the Professional On-Call of your situation, leave a message with your return number if necessary. The Professional On-Call will return your call within 1/2 hour. Please note, there will be a charge for after hours crisis calls.

What is Needed from You

Completing the Initial Online Appointment Request Form below, please include:

  • Your contact information: Name, Phone Number, e-mail Address
  • Best Day and Time for a return call 
  • Name of your insurance carrier(s) 
  • Referral source
  • Short, simple statement of the type of service you are requesting

 

Information Needed When You Call for an Appointment:

  • Insurance Card(s) - With Insurance ID Number and Customer Service Phone Number 
  • Date of Birth
  • Calendar  

 

Your Initial In-Office Appointment:

  • Confirm your New Patient Appointment by phone the day 48 hours (or 2 days) in advance of your appointment
  • Ask your medical doctor to FAX a referral request to our office prior to your first visit  (FAX Number: 850-243-2702) - having a referral speeds up your insurance authorization process by verifying that treatment is "medically necessary" 
  • Make sure you know how to locate our office in Mary Esther - Directions
  • Arrive 30 minutes before your scheduled appointment time
  • Bring - Insurance Card(s); A Photo ID, Your Medical/Referral Information; Prior Psychological Testing Results; School Records; Medication List; PCM Contact Information
  • Bring the completed New Patient Information Packet - Download the Initial Paperwork at Helpful Forms and complete at home before your appointment OR if Necessary, Complete the Packet in-office before your Initial Appointment
  • If being seen as a couple, each partner is requested to independently complete a New Patient Packet
  • Remember - Payment (such as a deductible, co-pays, or self-pay fees) will be due on the date of your first appointment and on each date of service thereafter.
